Upload Study Documents

Enter the research access code to upload signed packets and study documents into a subject's restricted folder.

Clinical Research

Upload study documents.

Scan and upload anytime after the visit — the patient does not need to be present. Files go directly into the subject's restricted Google Drive folder, filed by document type, and SpectOcular admin is notified automatically.

Upload

Upload the entire signed packet, not just signature pages.

Pick what you are uploading — it sets the document type below. Bookmark this page on the site computer.

Search by Subject ID on its own, or by name below. You do not need both.
Search by the Subject ID printed on the consent packet, or by name (at least two letters). Only patients at your site are searched. A single match is selected for you automatically.
Recorded with the document so it files against the correct visit.
PDFs up to 4 MB upload in one shot. Larger files — imaging DICOM/ZIP up to 512 MB — upload in parts automatically; keep this page open until it reports done.
Required for a signed consent packet. Enter the date written on the participant's signature line, not today's date unless that is the date signed. Cannot be in the future or before the packet was generated.
Where files go: each upload lands in the subject's restricted Google Drive folder, in a subfolder chosen by document type (signed consents, medical records, labs, imaging, Retina IQ reports), renamed with the Subject ID and visit, and logged in the study document register with who uploaded it and when.
